You can sign up in 4 easy steps:

• 1. Complete the Bluefin 5 Year Boilerguard application form enclosed with boiler.
• 2. Householder to sign declaration on the Bluefin 5 Year Boilerguard application form.
• 3. Complete Boiler Passport.
• 4. Complete the Installer Claim Form below and return with proof of purchase, completed Boiler Passport and completed Bluefin application form to Firebird.
